Navigating Your Journey to Independent Living
Managing your mental health care plan can be challenging. Through our community support services, we offer a comprehensive program that supports you throughout your treatment journey, especially during your transition to or from independent living at home.
As per the Department of Behavioral Health guidelines, Community Support programs include:
- Collaborating in the development and implementation of a Consumer’s IRP/IPC and Community Support Individualized Service Specific Plan (“ISSP”).
- Providing assistance and support during stressful situations.
- Offering mental health education, support, and consultation for individuals and their families.
- Delivering individual mental health services to develop interpersonal and community coping skills tailored for adapting to home, school, and work environments.
- Assisting in symptom self-monitoring and management to minimize the impact of psychiatric symptoms on daily living, financial management, personal development, or school/work performance.
- Helping to enhance social support skills and networks to alleviate life stresses stemming from mental illness and promote independent living.
- Developing strategies and supportive interventions to prevent out-of-home placement and strengthen family support skills and understanding of individuals’ strengths and limitations.
- Creating plans and strategies for preventing mental health relapses.
